The federal marking rule, 49 CFR 390.21, asks for the legal name or a single trade name of the carrier and its USDOT number preceded by the letters USDOT. They go on both sides of the vehicle, in letters that contrast sharply with the background and can be read from 50 feet in daylight while the vehicle is stopped. The rule gives no size in inches. Whether it covers your truck, and what else your state asks for, is yours to confirm with FMCSA or your state agency.
The name has to match your registration. The rule says the name as listed on your Form MCS-150, and 49 CFR 390.19T allows only the legal name or a single trade name on those forms. So type it exactly, with the same ending, such as LLC or Inc. Slogans, a second DBA and shortened nicknames are the usual mistakes. On the second line, type USDOT followed by the digits, as in USDOT 1234567.

The legal name or a single trade name of the carrier, as listed on its registration (Form MCS-150), per 49 CFR 390.21. If you operate under a DBA and that is the trade name on your registration, use that one. If your registration says one thing and your truck says another, ask FMCSA how to change the record before you order lettering.
If another company's name also appears on the vehicle, the rule says the operating carrier's name and USDOT number must follow it, preceded by the words "operated by." FMCSA's guidance on shared vehicles says the markings of the carrier responsible for the operation must be displayed. You can type OPERATED BY at the start of the name line if it fits in the preview.

The rule asks for letters that contrast sharply with the background. Because the decal is a solid rectangle in your chosen background color, your door paint does not change how it reads. Dark lettering on a light background, or white lettering on a dark one, are the straightforward pairings. Check the live preview, and avoid pairs that look similar in tone, such as red on blue.
The marking has to appear on both sides of the self-propelled vehicle, so a covered truck needs the name and number on each side. Each decal is one piece and priced per decal, so order two. Shipping is a flat $5.99 per order, and free on orders of $99 or more, counted before discounts.
