DOT number stickers are made-to-order vinyl decals that put your USDOT number on the truck door, and Work Decals sells them from $12.99 per decal. Pick the number-only decal if your company name is already on the door, or a longer layout if you want the name, MC number or city on the same sticker. Order two, one for each door, and apply them yourself.
The federal marking rule, 49 CFR 390.21, asks for the carrier's legal name or a single trade name, and its USDOT number preceded by the letters "USDOT", on both sides of the truck. The letters must contrast sharply with the background and be readable from 50 feet in daylight while the vehicle is stopped.
Nobody pre-approves a decal. It carries whatever you type, so you are responsible for the name and number. Rules change. Our plain-English display rules cover the rest, and FMCSA's Do I need a USDOT number? page says whether your truck needs one.

Choose a sticker background from White, Black, Yellow, Silver, Red or Blue, and lettering in Black, White, Red or Blue. The lettering sits on the sticker's own background, so that pair is the one to get right. Black on white or yellow, or white on black, are plain high-contrast choices; the door builder warns when a pair is hard to read.

Both sides means one decal per door, so order 2 per truck; the product pages start at quantity 2. The rule covers the self-propelled vehicle, not a towed trailer, per FMCSA's 2000 final rule, so a fleet needs a pair per truck. The rule does not set a spot on the door: see where to put the USDOT number.

Type the letters USDOT first, as in "USDOT 1234567", because the federal rule asks for the number preceded by those letters. DOT number stickers and USDOT decals are the same thing: lettering of the number FMCSA issued you, alongside your name.
We make each decal exactly as you enter it. If you notice a mistake right after ordering, use the contact form straight away. If the decal has not been made yet, we will try to correct or cancel the order. The guarantee does not cover a wrongly typed number printed as entered.
It depends on weight and use. Under the federal definition, a vehicle used in interstate commerce counts when its gross vehicle or combination weight rating is 10,001 pounds or more, among other tests. See do pickup trucks need DOT numbers, then confirm with FMCSA and your state.
Ask the carrier you lease to before ordering. FMCSA guidance says the markings of the carrier responsible for operating the vehicle must be displayed, with "operated by" before its name when other names appear. For property-carrying leases, 49 CFR 376.11 puts identification on the carrier.
