A three-line decal for a leased truck: LEASED TO on top, the carrier's name in the middle, and its USDOT number underneath. Know what this wording is before you buy. The federal marking rule, 49 CFR 390.21, uses the words "operated by" when another company's name is on the vehicle. A "leased to" line is a common practice that the rule's wording does not require, so it is your choice and not a rule's text. If you want the rule's own phrase, the Operated By decal has the same layout at the same price.
Start by asking the carrier you are leased to what lettering it wants on the door. The leasing rule at 49 CFR 376.11 puts the job of identifying leased property-carrying equipment under Part 390 on the carrier that acquires it, for as long as the lease runs. The federal marking rule says "operated by" when another company's name is on the vehicle, and its wording does not call for "leased to." A leased to line is a common practice, and we cannot say that your carrier or any agency requires it. Ask the carrier, and read FMCSA's shared-vehicle guidance if you are unsure what your truck must show.

OPERATED BY copies the phrase in the federal marking rule, while LEASED TO describes the arrangement between you and the carrier. Neither decal changes your registration or your lease. Pick the wording your carrier asks for, and type your own phrase into the top line if it wants something different.
The sample shows the carrier named on line two with its own number on line three, so the name and number should belong together. Confirm which carrier the lease points to, then copy both from that carrier's records or the SAFER lookup. We print what you type and do not check it, and a wrongly typed number is not covered by the guarantee.
Yes. The top line is free text up to 24 characters, so it can read whatever your carrier asks for. The second line takes the carrier name and the third takes the USDOT number. Because we print exactly what you enter, the live preview is your chance to check spelling and spacing before you pay. The usual slips are a dropped letter in the carrier name and a swapped digit.
Check first. For a rental agreement of 30 days or less, 49 CFR 390.21 lets the lessor's name and USDOT number appear instead, when the agreement meets the conditions listed there. Decals are made to order and cannot be resold, so ask the rental company what it allows on the vehicle before you stick anything to it.
