Two ways to fill the five lines. A freight truck: company name, city and state, USDOT number, MC number, phone. A service van: business name, trade or tagline, city, license wording if your trade board asks for it, phone. If the vehicle is a commercial motor vehicle, the legal name and USDOT number have to be on it, and 49 CFR 390.21 allows other identifying information, such as a phone line, if it is not inconsistent.
The preview shows how the lines are sized: line 1 prints largest, line 2 smaller, and lines 3 to 5 close to line 1. So use line 2 for a city or tagline and put the phone number on line 5, where it prints larger. Then stand back from your screen and shorten any line that is hard to read.

For a service vehicle without a USDOT number, try the business name on line 1, the trade and city on line 2, a licensed and insured note on line 3, a website on line 4 and your phone number on line 5. Only line 1 is required. Lines 2 to 5 are optional, and the lines you do fill are spaced to suit. The price is $45 whichever lines you use, so fill all five if you have a use for them. If your vehicle must carry the federal marking, keep the legal name and USDOT number among the lines you fill.
The form allows up to 32 characters on lines 1, 2 and 5 and up to 24 on lines 3 and 4, spaces included. Longer text means smaller letters, because every line must fit the decal's width, so a 24 inch width gives long lines more room than a 12 inch one. Abbreviate a website or tagline rather than the legal name, which should match your registration if your vehicle must show it.
The five-line set is a fixed layout at a flat $45: name, city, USDOT, MC and one spare line. The door builder prices each line separately and offers extras, including an arched company name and room for a sixth line. Choose the builder when you need one of those. Choose this set when five plain stacked lines are what you want and you would rather type them into one page.
The marking rule's test is daylight legibility from 50 feet while the vehicle is stopped, and it sets no height in inches. With five lines each line gets less room than on a one-line decal, so width matters more: a 24 inch decal gives every line more room than a 12 inch one.
