A unit number is your own filing system, not a federal marking. The marking rule covers the self-propelled vehicle, and FMCSA's 2000 rule did not extend it to towed trailers. So a trailer carries a unit number because you want to find it quickly, while the USDOT lettering stays on the truck or tractor. The trailer rules guide has the details.
The second line is GVWR, the maximum loaded weight the manufacturer assigns to a vehicle, as defined in 49 CFR 390.5T. Type the manufacturer's rating for that unit, not what it weighed at the scale last week. A truck and its trailer each have their own rating, so each unit's decal carries its own figure, for example GVWR 26000.

Keep it short and consistent. Use a letter for the type and a number that is never reused, such as T-12 for a trailer and B-3 for a box truck, or the UNIT 07 style shown in the form. Short numbers are easier to read from the cab. Write the scheme down before you order, because changing it later means new decals for every unit.
A short unit number is quick for drivers and the yard, while a VIN is exact and never repeats. One approach is to put the unit number on the decal and keep the VIN in your records. If you want the VIN itself on the unit, the VIN decal carries it. Either way, pick one scheme per fleet so the decals look the same from unit to unit.
The federal marking rule applies to the power unit and does not fix a position on the side, so the spot is your choice. Use the same place on every unit, on both sides, so anyone looking along a row of trailers knows where to read. Keep the number clear of door seams and handles, and check it from across the yard.
Yes. The GVWR line is optional in the form, and the federal marking rule asks for a name and USDOT number, not a weight. Add GVWR when someone you work with asks for it, or when your state has a weight-marking rule. Some states do, and they differ, so check with your state agency first.
